Therapeutic Riding 2022 Registration:
Clients are added to the schedule as openings occur. Once a riding time has been set, the required forms must be completed. PDFs are available on this page, but we recommend you use our online signup forms instead. Paperwork for all prospective clients must be filled out in its entirety before a client is able to participate in a Main Stay program.

All new therapeutic riding clients must have a doctor's release and be evaluated by Main Stay's staff. Main Stay reserves the right to decline any client who we feel we cannot provide services to safely, or who is contraindicated in accordance with PATH, Intl. standards.
Required Therapeutic Riding Forms For Participation:
- Client Registration Form (Online)
- Session Payment Form (Online)
- Medical History and Physician’s Statement Form - to be printed and mailed/emailed/faxed to your doctor. This form usually takes the longest to receive back. Please send that immediately to your physician.
